At your first appointment, you will fill out some simple forms telling us about yourself, your medical history, your hearing loss and your insurance coverage. If you wish, you can fill out some of these forms at home and bring them to the appointment. Please also bring the following applicable information with you:
- Insurance card
- Referral for your hearing evaluation from your primary care physician (if required by your insurance provider)
- Your payment/co-payment
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
After the forms are completed, we will discuss your hearing concerns, conduct a comprehensive audiological evaluation and determine possible solutions. The appointment will take about an hour.
